Integration of Medical and Social Care-Evolution and Results
Is premature death one of the clearest ways to look at health outcomes? Research has shown that only 10% of premature deaths relate to healthcare, whereas 15% are related to social factors and a whopping 40% relate to health-related behaviors.
Community-based organizations offer effective, evidence-based interventions to address the 55% of health outcomes related to social and behavioral factors. Community agencies deploy interventions like in-home care transition coaching, health self-management education, or in-depth psychosocial and functional assessment in the home with service coordination and medication safety and fall-prevention activities. Across the country, community-based organizations are forming provider networks to make it easy for healthcare entities to access these powerful interventions, which bring a substantial positive return on investment. June Simmons, President and CEO of the Partners in Care Foundation is nationally recognized for her leadership in the administration of health care organizations. Since founding Partners in Care in 1997, June has led the organization’s pioneering role in developing and scaling evidence-based care models, leading to innovative and effective interventions for the management of chronic conditions, the challenges of aging, and better coordinated care to improve health care outcomes. June has been active as one of the co-founders of the national Evidence-Based Leadership Council. She continues to work in co-founding collaborations with other key leaders, most recently the Los Angeles Alliance for Community Health and Aging – a new public health partnership, designed to disseminate evidence-based intervention and education programs to achieve better health results for diverse populations. In Fall 2015, she joined the Board of the National Council on Aging (NCOA) and has recently completed terms on the National Advisory Council of the National Institute on Aging (NIA), and the Institute of Medicine (IOM) Committee on Transforming End-of-Life Care.
Sandy Atkins is the Vice President for Strategic Initiatives at the Partners in Care Foundation. She is the key contact for Partners’ nationally-recognized HomeMeds℠ program, and works closely with Partners’ CEO, June Simmons, to provide strategic guidance to the organization. Prior to joining Partners, Sandy was Program Director of USC’s Center for Long-Term Care Integration.
